IT Request - New Hires
In order to ensure that all new hires have the hardware and software they need to start work, a minimum of two weeks prior notice is required. A greater advanced notice and in person meeting is needed if the position requires specialized software, equipment, or new systems.
IT Request - Unplanned Terminations
IT must be notified of any employee that has computer systems access and is unexpectedly terminated or quits. This is for security reasons to protect employees and Game Gear. Failure to do so exposes everyone to great risk.
IT Request - Planned Employee Departures
Two weeks notice is preferred and will allow IT to schedule the termination request without any delays. Immediate notification must be given to the IT department if the employee leaves before the date and time that you provide on this form, this will allow IT to follow the proper security procedures.
